Financial Aid Forms for the 2020-2021 academic year
Below is the list of documents for the Fall 2020 - Summer 2021 Academic Year. These forms are valid only these semesters. Do not submit these forms for the Fall 2019 - Summer 2020 academic year.
Newly admitted students may submit documents through Dominican's Financial Aid Portal or by mail/in-person. All items must be physically signed and dated by the student and a parent (if applicable) unless otherwise noted.
ANNOUNCEMENT FOR ALL CURRENTLY ENROLLED STUDENTS
With the exception of certain appeal forms, all currently enrolled students must submit any required forms through DUrForms. Create an account on DUrForms only if instructed to do so. If you have any questions, contact the Office of Financial Aid.
