Audio Visual Equipment Request

This form is for requesting equipment only. Until your request has been confirmed, this is not a guarantee of this equipment. You will receive a call or email from the Library Circulation Desk within one business day regarding your request.

You do not need to reserve an LCD Projector or Laptop Computer if you are in a Smart Classroom.
The Smart Classrooms are: GUZ 104, GUZ 110, GUZ 112, GUZ 114, GUZ 201, GUZ 307, LIBR 201, and LIBR 204.

If you encounter any problems using this form, send an email directly to circdesk@dominican.edu with all of the required information.
