Follow these CTC instructions - TB Test Clinics
TB tests can be scheduled at local clinics: The Mendocino Community Health Clinic is located at 333 Laws Avenue, Ukiah, CA 95482. Call at 707-468-1010. The Lake County Health Clinic is located at 922 Bevins Court, Kaleport, CA. 707-263-1090. - Federal Student Aid
Complete FAFSA online at www.fafsa.ed.gov and enter the Dominican School Code #001196 to ensure the Student Aid Report is sent to Dominican University. - California State Assumption Program of Loans for Education (APLE)
