Course Evaluation Instructions

The procedures for course evaluations have changed. For the evaluations to be tabulated, it is essential that you follow the instructions below precisely.

- Each student in your class should receive an evaluation form.
- The ASDU has requested that you distribute forms at the beginning of the class
- Write the following information on the board:
- Instructor’s last name, first initial
- Call number
- Discipline (i.e., BIO, ENGL, ARTH)
- Course number (i.e., 1500, 3501)
This information needs to be recorded correctly on the form.
PLEASE WORK WITH THE STUDENTS TO FILL OUT THESE FIELDS ON THE FORM BEFORE YOU LEAVE THE ROOM. - Stress the importance of filling in boxes completely (not with checkmarks) using a #2 pencil. If the boxes are not filled in correctly, the form cannot be scanned.
- Explain the purpose of the course evaluations and assure the students that their confidentiality will be respected. Instructors will not see handwritten comments.
- Leave the room and allow at least 10 minutes for students to fill out the forms.
- One student should be designated to collect the completed forms, put them in the envelope along with any blank forms, and return the envelope to the Provost’s Office, Guzman 210. (After 5:30 p.m. they may be slipped under the door of Guzman 210 or left with the circulation desk at the Library.)
- If a student is absent on the day that forms are distributed, he/she may obtain one from the Provost’s Office, Guzman 210. Return to Laverne Dicker in the Provost’s Office.
After grades are in, reports summarizing the responses will be sent to you by your Dean’s Office. You may respond in writing to any criticism, and this response will be kept with the evaluation forms.
If you need additional pencils, you may obtain them from the Office of the Provost, Guzman 210.
Please return blank forms in the envelope for future use.
Please note: There are two opportunities for you to ask additional questions of students on side #1. You may wish to give some thought to these opportunities before distributing the evaluations. Be advised that the form already asks these questions: “What did you like best about this course and this teacher? What suggestions do you have for this course and this teacher?”
